Side-Effect Reporting page

Purpose: tell patients what to do if something goes wrong and how to report it. Typical content:

  1. Emergency first. For severe symptoms (trouble breathing, swelling of face or throat, chest pain, fainting, etc.), call 911. For poisoning or overdose, call Poison Control at 1-800-222-1222.
  2. Contact your clinician. For non-emergency side effects, stop and contact your treating clinician or use the support email and phone. Don’t change dose on your own.
  3. Report to Ridgewell. A support email or form. Ask for: name, order number, medication, date started, what happened, and when. Say the report will be shared with Partell and the clinician as needed.
  4. Report to the FDA. Patients can report adverse events to FDA MedWatch at 1-800-FDA-1088 or fda.gov/medwatch. This applies to compounded drugs too.
  5. Product problems. Damaged, wrong, or suspicious packaging: don’t use it, and contact Ridgewell and Partell (details are on the medication label).
  6. What happens after you report. Reports are reviewed, and clinical concerns are routed to the clinician.
  7. Disclaimer. The page isn’t medical advice, and reporting doesn’t replace urgent care.
